Responsibilities: Works collaboratively with the RN Case Manager to provide discharge planning based on assessment of patient and family needs, preferences and available resources in order to ensure timely discharge and provide appropriate linkage to post-discharge care providers (IE New/resumptions SNF, LTAC, Rehab, Hospice, and other Community resources) Investigates cases admitted with 'no insurance' or 'self-pay' to identify whether insurance coverage does exist, and if the need for financial assistance is warranted. The social worker provides psychosocial assessments, supportive counseling, crisis intervention, community resource connection, financial resources and works collaboratively with all other members of the healthcare team, ensuring clear communication and care planning to all patients.
