Care Manager Responsibilities: • Provide case/care management to Medicaid Members seeking assistance from Salvation Army Centers, including screening, assessment, social care plan, and direct service delivery related to specific health-related social needs as identified by Social Care Network. Senior will also provide Care Management and is responsible for providing service to the Member throughout the social care journey including conducting screening, eligibility assessments, managing referrals, completing direct assistance and follow-ups to address outstanding needs.
