Actively participates in the coordination of care services, evaluating the ongoing needs of residents (attends resident care plan meetings, rehabilitation and resident/family meetings, departmental staff meetings, annual policy review meetings, monthly quality assurance meetings, etc.). Establishes and maintains positive relationships with residents, family members, physicians, community leaders, state and federal surveyors, hospital case managers and other affiliated professionals.
