Supports Coordinators will follow regulation guidelines by Office of Developmental Programs (ODP) and meet monitoring requirements based on their funding level: SC Services Only, Base, PFDS, Community Living Waiver, or Consolidated Waiver. Locating services and supports consists of assistance to the participant and his or her family in locating, arranging, and obtaining services-including needed medical, social, habilitation, education, employment, transportation, and other needed community services.
