Care Management Social Work services include but are not limited to the following: biopsychosocial assessments, crisis intervention, fetal or reproductive loss, end of life, grief and loss, assessment of coping, medical literacy, abuse assessment and referral to CPS, accessing resources to facilitate hospital transitions and/or discharges, adoption coordination, linkage to community agencies, mental health assessments, tangible resources, participation in team meetings and educational activities related to the needs of patients and their families.
