Help maintain clinical continuity by identifying gaps or barriers in treatment flow, such as but not limited to signed plan of care, authorizations for services, need for reassessments, expiring plan of care, and communicating needs to the Site Manager/Headquarters. Serve as an on-site clinical resource for the Site Manager and therapists, offering guidance on treatment logistics, clinical tools, session planning needs, and patient-care coordination.
