Completes supporting documentation in an accurate and timely manner, including but not limited to; visit report, OASIS assessment, care plan/plan of care revisions, medication profile, progress notes, revisions to the plan of treatment for recertification, discharge/transfer summary, etc. Provides frequent communication to the RN Team Leader, physicians, other team members, etc., and the patient/family in efforts to coordinate changes in patient care and assures appropriate follow-up and supporting documentation is done.
