p>POSITION SUMMARY: Under the supervision of the director of specialty pharmacy and clinical development, and according to department policies, procedures, and guidelines, the Specialty Clinic Pharmacist is responsible for the enrollment and ongoing prescription adherence monitoring for all patients referred by pharmacy liaisons for Specialty Care Management, responsible for comprehensive medication reconciliation with electronic medical record of each patient during prescription verification.
ESSENTIAL RESPONSIBILITIES / DUTIES:
The Specialty Clinic Pharmacist plays a key role in ensuring safe, effective, and accessible specialty medication therapy for patients with complex and chronic conditions through direct patient care as outlined in the scope of practice.
