POSITION SUMMARY: This position is responsible for supporting the Registered Nurse with assessments of client physical and mental health needs, planning all medical interventions for inclusion in the treatment plan, facilitating health / wellness education groups, medication monitoring, advocacy as appropriate, and the overall surveillance and maintenance of the health needs of clients.
