Responsibilities: Communicate and document appropriately and effectively with supervisors, physicians, nurse practitioners, unit based pharmacists, social workers, nurse managers, peers, patients/customers and outside agencies. Summary: Under Pharmacist supervision, the Pharmacy Technician Care Coordinator communicates with nurses, physicians, pharmacists, and other members of the medical team to assist patients with medication access and adherence.
