The heart transplant multidisciplinary team includes heart failure/heart transplant physicians, cardiac surgeons, heart transplant APP and RN's and transplant social work, pharmacists, nutrition, financial, physical therapy and patient service coordinators. The multidisciplinary team includes integrated care coordination with a variety of specialized consultants including transplant infectious disease, transplant psychiatry, endocrinology, hepatology, nephrology, pulmonology, histocompatibility, cardiac pathology, cardiac rehabilitation, etc.
