Key ResponsibilitiesAssist clients in securing and maintaining stable housingHelp clients apply for and maintain benefits including SSI, SSDI, Medicaid, SNAP, and other entitlement programsConnect clients to community resources, transportation, employment services, food assistance, and other supportsProvide life-skills coaching related to budgeting, meal preparation, hygiene, transportation, and independent livingAdvocate for clients with housing providers, community agencies, healthcare systems, and government programsCoordinate services with therapists, nurses, psychiatrists, peer support specialists, and community partnersSupport clients involved with the legal system, including probation, parole, and court programsConduct community-based visits in homes, shelters, and other locationsParticipate in daily interdisciplinary team meetingsMaintain accurate and timely documentation in the Electronic Medical Record (EMR)Participate in rotating weekday evening on-call coverage.
