p>Duties may include, but are not limited to the following: - Provides public health nursing services in a variety of settings, such as homes, clinics, programs, and the community;
- Performs health assessments and intervention plans for population groups and geographical areas;
- Identifies and evaluates both systems and individuals, medical, psychosocial, and socio-economic barriers to optimal health and well-being and takes the appropriate steps to mitigate their impact;
- Collaborates with other agencies and community-based organizations on behalf of individual, family, and community health promotion;
- Recognizes multicultural practices and beliefs, and their impact on health behaviors;
- Develops, implements, and evaluates nursing care plans based on identified individual, family or community needs, strengths, and availability of resources;
- Refers individuals and families to community partners based on identified bio-psychosocial needs;
- Responds to public health emergencies and disasters as needed, utilizing the nursing skills authorized under their professional scope of practice;
- Travels to client homes, community events, and various work locations; and.
LATERAL TRANSFER OPTION: If presently permanently employed in another "merit" or "civil service" public agency/entity in the same or substantively similar position as is advertised, and if appointed to that position by successful performance in a "merit" or "civil service" style examination, then appointment by "Lateral Transfer" may be possible.
