Description Summary:Centralized Scheduling Representatives prepare paperwork for the medical chart and billing folder. Determines financial status and obtains requirements for self-pay patients. Obtain and appropriately route the orders received from various physicians, patients, and Document Delivery. Adhere to a high standard of customer service. Schedulers will perform all other duties as assigned to promote efficient and effective operations. Enter all demographic information from patients, physician offices, posting sheets, Document Delivery, etc., when scheduling procedures. Schedulers are the dispositions for Document Delivery. The schedulers will monitor the documents that are faxed into Document Delivery and are required to disposition them in the appropriate queues for scheduling. Required to email all late add-ons or changes to the schedule to all appropriate parties. Responsibilities:Scheduling:<ul><li>Obtains and accurately inputs all required data elements for scheduling, including patient demographic, financial information, and relevant notes associated with the encounter.</li><li>Data fields include but are not limited to: Patient Phone number, insurance plan, written admitting diagnosis, working diagnosis, and physician information.</li><li>Prioritizes and completes scheduling in a consistent, courteous, professional, accurate, and timely manner.</li><li>Ensures each patient is assigned only one medical record number.</li><li>Selects appropriate patient type based on the department and services required.</li><li>Documents in account notes.</li><li>Ensures orders are received and are consistent with tests/procedures.</li><li>Prepares account/patient folder with necessary forms completed and signed.</li><li>Gives patient documents that he/she needs to take with him/her to other departments.</li></ul>Document Delivery:<ul><li>Ensure documents are dispositioned accurately.</li><li>Access appropriate documents necessary to complete the scheduling process.</li><li>Monitor documents to ensure all procedures are scheduled.</li><li>All procedures will be scheduled daily.</li><li>Demonstrates accuracy in selecting insurance plans (I-plans).</li></ul>Customer Focus:<ul><li>Effectively meets customer needs, builds productive customer relationships, and takes responsibility for customer satisfaction and loyalty.</li><li>Answers phone patients in a courteous and professional manner.</li><li>Calls patients by name.</li><li>Asks patients if they may have special needs.</li><li>Represents the Patient Access / Pre-Access/Centralized Scheduling department in a professional, courteous manner at all times.</li></ul>Errors:<ul><li>Makes minimal errors in performing admissions / scheduling / insurance verification / pre-registration. See Error Policy and Procedure for target error rate percentage.</li><li>Utilizes education information to reduce error rates.</li><li>Requests additional education information when necessary.</li><li>Demonstrates ability to select correct insurance plans.</li></ul>Other:<ul><li>Required to assist the hospital in the event of an internal or external disaster.</li><li>Supports the flexible needs of the department to accommodate patient volume in all areas of the hospital. Description <div>CHRISTUS Health, as a recipient of grant funding, partners with AmeriCorps State and National to provide outreach, education, and navigation services to address the social determinants of health across each of the communities we serve. Significant goals of this partnership are to: 1) Serve communities with concentrated poverty, rural communities, and tribal communities; 2) Create workforce pathways for AmeriCorps members, including deliberate training, such as pre-apprenticeship and apprenticeship opportunities, certifications, and hiring preferences or support; and 3) Enhance member experience by providing opportunities for skill attainment, personal growth, and connections to the community they are serving in support of a lifetime of civic participation. </div><div>To this end, CHRISTUS Health will engage with individuals to serve their communities as AmeriCorps service members. Members serve for a minimum of a one-year term and will not be considered employees of CHRISTUS Health. Members have the opportunity to receive Community Health Worker training and obtain state certification. The role includes consolidating programmatic data and sharing insights with leadership. </div><div>The Community Connector II delivers culturally competent, patient‑centered screening, referral, and navigation services to connect individuals and families with appropriate social and medical resources. This position supports individuals in developing care plans, identifying barriers, setting goals, and accessing needed services.
