These services include assessments, comprehensive service plans, and linkage to resources to help clients and their families live healthy, productive lives.</p><p>Under the general direction of the Program Director, the Outreach Coordinator and Systems Manager (OCSM) will collect, analyze, monitor, and report data related to the Hope Care Management Health Homes program. The OCSM will combine diverse data sets, report findings to leadership, and oversee outreach and intake staff, supporting the mission of Samaritan Daytop Village.</p><h3>Responsibilities</h3><p><strong>What You Will Do</strong></p><ul><li>Analyze data and processes to improve reporting and support program objectives.</li><li>Create user accounts for various systems such as PACT, GSI, RMA, Epaces, and HCS.</li><li>Conduct bi-weekly PC-MVS reports for Care Coordination Supervisors and Program Director.</li><li>Evaluate encounter data for errors and discrepancies.</li><li>Distribute utilization data to address client needs.</li><li>Coordinate with Lead Health Homes and Managed Care Organizations to improve outcomes.</li><li>Review reports for discrepancies and report issues to the Program Director.</li><li>Attend relevant department, DOH, and partner meetings.</li><li>Collaborate with team members to identify challenges and goals.</li><li>Provide training to new staff members as needed.</li></ul><h3>Qualifications</h3><p><strong>Who You Will Be</strong></p><ul><li>BA in Accounting, Finance, Business Administration, or related field.</li><li>2-3 years' experience in billing and health program coordination.</li><li>Experience with Health Home or Care Coordination programs.</li><li>Proficiency in Microsoft Office and EHR systems.</li><li>Experience working with diverse populations.</li><li>Strong interpersonal, organizational, analytical, and problem-solving skills.</li><li>Attention to detail and accuracy.</li><li>Knowledge of office administration and billing procedures.</li><li>Ability to work independently and adapt to change.</li><li>Maintain confidentiality of client information.</li><li>Leadership skills to supervise and train staff.</li></ul>#J-18808-Ljbffr
