In addition to clinical responsibilities, this individual will serve as a champion for the hospice and palliative care programs, supporting outreach and relationship-building with physicians, referral sources, discharge planners, hospital teams, and community organizations to promote awareness, education, and access to hospice and palliative care services. Clinical responsibilities include providing hospice medical oversight, conducting goals-of-care discussions, symptom management, and delivering inpatient palliative care consultation services for hospitalized patients with complex medical and supportive care needs.
