This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. 1 Weeknight and 1 Weekend Needed! $5,000 Sign On Bonus!!! About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role. Weekend Hours- Friday 5pm- Monday at 5pm Essential Functions<ul><li>Ensure appropriate delivery of hospice services to eligible patients and families.</li><li>Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges.</li><li>Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs.</li><li>Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication.</li><li>Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members.</li><li>Educate patients and families on disease progression, symptom management, medications, and hospice philosophy.</li><li>Communicate significant changes in patient condition promptly to hospice and attending physicians.</li><li>Maintain accurate, timely clinical documentation including assessments, care plans, and notes.</li><li>Assist with rescheduled, missed, or reassigned visit requests based on triage direction.</li><li>Participate in calendar and scheduling modifications, including service codes and clinician assignments.</li><li>Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection.</li><li>Assist with TPE reviews, QAPI efforts, and regulatory compliance.</li><li>Ensure infection control standards and universal precautions are consistently followed.</li><li>Provide leadership and guidance to field staff; conduct supervisory visits when appropriate.</li><li>Participate in required meetings, after-hours updates, and trainings.</li><li>Maintain proficiency in pain and symptom management for hospice and end-of-life care.</li><li>Promote the organization’s core values and maintain a culture of compassion, professionalism, and teamwork.</li><li>Perform other duties as assigned.</li></ul>

Experienced Machine Operator – Weekend Shift (Nights)   Position Snapshot An experienced machine operator is commonly expected to know how his/her equipment operates and be able to identify and repair simple mechanical problems like stalls or jams. Workers hired into this position will be trained to eventually operate various machines and learn processes related to manufacturing steel wire products.
