General Summary of Position
MedStar Health is looking for a Behavioral Health Coordinator for the Care coordination and case management for the Behavioral Health team in the Department of Family Medicine. The Behavioral Health Coordinator Plans and coordinates disposition services for patients with psychiatric concerns or illness, health psychology, counseling, group, family, and individual therapy needs. Manages the registry of behavioral health patients. Presents clinical information to agencies or outside referrals to coordinate care and disposition, Behavioral as well as to insurance companies for clinical review for authorization.

Primary Duties:
Coordinate admissions, referrals, scheduling, and patient access for Behavioral Health Services, ensuring timely enrollment and continuity of care.
Maintain accurate patient records, tracking referrals, authorizations, admissions, discharges, and service utilization while ensuring compliance with documentation standards and regulatory requirements.
Serve as a liaison between patients, providers, community agencies, referral sources, and third-party payors to facilitate care coordination and access to behavioral health resources.
Mnitor and report patient metrics, social determinants of health, and program outcomes, identifying care gaps and coordinating appropriate follow-up services.
Support quality improvement initiatives, multidisciplinary teams, and community outreach efforts while maintaining compliance with Behavioral Health Integration, Community Health Integration, and care coordination program requirements.
Qualifications:
Bachelor's degree in psychology, Social Work, Occupational Therapy, Psychiatric Nursing or other mental health discipline or an equivalent combination of education and related experience. (Required)
Master's degree in In Social Work, LMSW, passed the ASWB Master's Exam (Required)
Licensed Certified Social Worker passed the ASWB Advanced Generalist Exam. (Preferred)
3-4 years 2 years of supervised social work experience after LMSW, including but not limited to case management, care coordination, program development, program evaluation, policy, administration, advocacy, or some counseling under supervision.
