Case Manager

Duties :
- Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process Performs utilization management and quality screenings In- Patient Case Management is required .

Skills :
- 2 years of acute care experience strongly preferred
- Case Management, Managed Care. In Patient Case Management is required

Education :
- BSN required
- RN license required
- CCMC preferred
