Responsibilities/Job Description:

Fairview is seeking a Certified Diabetes Educator to join our team at M Health Fairview Clinics & Surgery Center. This is a 0.8FTE (64hours/2 week pay period) on the Day shift, 7:30am-4:00pm or 8:00am-4:30pm.

 

The Endocrinology Clinic provides a wide range of services for endocrine needs including subspecialty programs for diabetes, thyroid disease, and reproductive health patients. Our dedicated team of providers, nurses, social workers, pharmacists, and other health care givers care for the whole patient and use resources from the community to assist with ensuring the patient has what they need to improve their health. 

 

Manages patients to meet chronic disease management goals. Interacts with patients and families in a patient centered manner. Works toward achievement of targeted outcomes in collaboration with the members of the healthcare team. Strives to activate patients/families to be more accountable for their healthcare maintenance. Educates patients and families on chronic disease management, preventive care management and acute care management. Assists team with development of related policies and programs. Consults with physicians and/or dietician on unusual or abnormal cases. Experienced in the care of diabetes patients to assess, plan, deliver, and evaluate nursing care for assigned patient population. 

Responsibilities

  • Interacts with patients and families in a patient centered manner. Uses effective communication strategies to achieve desirable outcomes.
  • Strives to activate patients and their families to be more accountable for their healthcare maintenance. Assesses patient needs and identifies expected outcomes.
  • Works toward achievement of targeted outcomes in collaboration with the members of the healthcare team. Implements nursing interventions based on plan of care.
  • Evaluates care and the patient’s response to interventions and expected outcomes.
  • Delivers care as delegated by physician/authorized practitioner to assigned patient population, or individual patient.


Required Qualifications

  • A.A./A.S.
  • 3 years experience with pediatrics and adult diabetics an ambulatory care setting.
  • 2 years experience in specialty area.
  • Basic Life Support (American Heart Assoc or Red Cross)
  • Diabetic Educator CDE Certification
