Schedule: Full Time | 40 hours per week | 8:00am-4:30pm | Weekend & Holiday Rotations, Required Department: Thome PACE | Jackson, MI | Day-Center & In-Home Care Travel Requirements: This role involves regular travel between Jackson, Lenawee, and Hillsdale counties, providing Home-Based Care. On-Call Responsibilities: Participate in after-hour on-call rotations, including evenings, weekends, and holidays.
What You’ll Do:
As a Certified Nurse Aide (CNA) in our Day Center and providing in-home care, you'll play a vital role in ensuring our participants receive the highest quality care. From assisting with daily activities to providing comfort and companionship, your work will make a meaningful impact on the lives of those we serve in Jackson, Lenawee and Hillsdale counties.
Key Responsibilities
Provide Hands-On Care – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting.
Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.
Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials.
Provide Responsive Support – Respond with kindness and compassion.
Monitor with Watchful Eyes – Report any changes in conditions to the leadership ASAP.
On-Call Responsibilities: Participate in after-hour on-call rotations, including evenings, weekends, and holidays.
Rotation Schedule: Be available for evening and weekend shifts as part of the rotation.

Skills, Knowledge & Expertise
A current and valid Michigan CNA Certification, required.
CPR Certified or willing to obtain.
Six (6) months of clinical experience as a licensed CENA, preferred.
Prior home care experience, preferred.
The ability to read, write, understand, and follow directions.
Physical ability to lift, stand, bend, transfer, stretch, walk, push, and pull as needed.
Ability to lift a minimum of 35 pounds frequently is required.
