Responsibilities:

• Develop and execute community outreach strategies to identify and recruit eligible patients
• Build and maintain relationships with community organizations, healthcare providers, and local partners
• Conduct outreach activities at community events, health fairs, and partner locations
• Screen and enroll prospective patients into programs, ensuring eligibility requirements are met
• Maintain accurate records of outreach activities, patient contacts, and recruitment metrics
• Serve as a trusted resource and point of contact for patients navigating available services
• Collaborate with internal teams to align outreach efforts with organizational goals

Requirements:

• Experience in community outreach, patient recruitment, or a related healthcare/social services role
• Strong interpersonal and communication skills with the ability to connect with diverse populations
• Knowledge of local Glendale/Phoenix Metro community resources and healthcare landscape is a plus
• Ability to work independently, manage time effectively, and meet recruitment goals
• Proficiency in maintaining accurate documentation and data entry
• Bilingual (English/Spanish) preferred but not required
• Valid driver's license and reliable transportation for community-based activities
