Key Responsibilities:
- Deliver physical therapy services to patients in their homes, including assessments, treatment plans, and interventions tailored to each individual’s needs.
- Evaluate the home environment and recommend modifications or resources to improve safety and mobility.
- Monitor patient progress and communicate updates or changes in condition to physicians.
- Collaborate with caregivers, nurses, and other healthcare professionals to ensure coordinated, comprehensive care.
- Refer patients to additional home health services when appropriate.
- Participate in discharge planning and ensure continuity of care.
- Supervise Physical Therapy Assistants in accordance with company policies and state regulations.
Qualifications:
- Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
- Current Physical Therapy license in the state of Pennsylvania.
- Minimum of 2 years of experience as a Physical Therapist; home health or community-based care experience preferred.
- Valid CPR certification.
- Reliable transportation and a valid driver’s license.
