Location: Grand Junction, CO 81506
Shift: Variable Night Shift (12-hour shifts)
Hours: 36 hours per week

Position Summary

"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care in a skilled nursing and long-term care setting. This role focuses on delivering safe, patient-centered care during overnight shifts, including medication administration, clinical monitoring, and coordination with the interdisciplinary care team.


Key Responsibilities

  • Administer medications and treatments as prescribed by physicians
  • Monitor residents’ conditions and promptly report any changes
  • Perform routine nursing assessments and accurately document care provided
  • Provide wound care and other basic nursing procedures within scope of practice
  • Maintain complete and accurate clinical documentation and charting
  • Assist with admissions, discharges, and transfers as needed
  • Support Certified Nursing Assistants (CNAs) in resident care activities when required
  • Ensure adherence to infection control and safety protocols at all times
  • Respond to emergencies and resident needs during night shifts
  • Collaborate effectively with the interdisciplinary healthcare team

Required Qualifications

  • Active and unrestricted LPN license (Colorado or compact state eligibility)
  • Prior experience in long-term care or skilled nursing preferred
  • Ability to work consistent overnight 12-hour shifts
  • Strong clinical judgment and documentation skills
  • Ability to work independently and manage patient care during night shifts
