Collaborates with physicians, behavioral health providers, social workers, and public health nurses to provide integrated, wraparound care ensures appropriate referral pathways are followed, including both intra-organizational resources and external community providers, to optimize patient outcomes. - Effectively uses electronic health information retrieval systems, data-collection processes, and technology applications for patient's records, fiscal management, tracking, reporting, and maintaining information for individuals, populations and/or program needs to assist with program development, grant-funded projects, and quality improvement activities as needed.
