Responsibilities for this position include, but are not limited to:
The LPN Care Manager will support the Transitions of Care Program, which focuses on reducing hospital readmissions within 30 days. The LPN will work under the direction of an RN Care Manager to implement care plans, coordinate follow-up services, educate patients, and promote continuity of care. This is primarily a remote and telephonic care coordination role, not a hands-on nursing position.
Duties:
Coordinate patient care in collaboration with the RN and multidisciplinary care team.
Schedule follow-up appointments and identify or address barriers to accessing care.
Monitor and document patient biometric data and screening results.
Provide and reinforce patient education regarding diagnoses, medication adherence, and self-management.
Conduct screening calls, complete assessment documentation, and assist with triage.
Maintain frequent contact with patients to monitor progress and support continuity of care.
Attend patient visits and complete home visits when needed for observation, education, or care coordination.
Connect patients with appropriate community resources and support services.
Participate in virtual care team rounds with RNs, social workers, pharmacists, and other healthcare professionals.
Provide patient updates and document all interactions accurately in the electronic health record (EHR).
Assist the RN with implementing the plan of care (POC) and monitoring patient goals.
Manage a rotating caseload of approximately 30–40 patients per month.
Support the 30-day transitions-of-care program, reassessing patient goals and determining whether patients can be discharged from the program or transitioned to long-term care management.
Perform additional duties as assigned by the RN.
Requirements
JOB REQUIREMENTS:
Active LPN license Required
Bls
First-time travelers are accepted.
Local candidates are accepted, with a lower rate for candidates residing within 75 miles.
Minimum of 1 year of experience required in home health or mobile case management.
