We are seeking a compassionate and experienced Nurse Practitioner (NP) to join the PACE (Program of All-Inclusive Care for the Elderly) team in Grand Junction, Colorado. This rewarding contract opportunity allows you to provide comprehensive primary care services to seniors while helping them remain healthy, independent, and living safely within their communities.
Working alongside the Chief Medical Director and an interdisciplinary care team, you'll deliver patient-centered care in the clinic, PACE Day Center, and participants' homes.
