Work Schedule: Day 5x8-Hour (08:00 - 16:30) Duration: 13 weeks Hours: 40 hours/week Pay rate: $40.00 - $48.00 /hr
Responsibilities for this position include, but are not limited to:
Coordinate patient care under the direction of the RN Care Manager.
Conduct follow-up calls to assess patient needs and support continuity of care after hospital discharge.
Assist patients with scheduling follow-up appointments and addressing barriers to accessing care.
Reinforce patient education regarding medication adherence, treatment plans, and ongoing health management.
Monitor biometric data and communicate concerning changes in patient status to the appropriate clinical team.
Support medication reconciliation and coordinate prescription refill requests.
Connect patients with community resources, pharmacies, home health agencies, and other healthcare services.
Participate in virtual multidisciplinary care team meetings with RNs, social workers, pharmacists, and other healthcare professionals.
Document patient interactions, care coordination activities, and updates in the electronic health record (EHR).
Support care plan implementation and help evaluate patient progress during the transitions-of-care program.
Conduct patient visits as needed, including potential visits to homes, hospitals, and provider offices.
Manage an assigned patient caseload and work independently while maintaining clear communication with the care team.
Requirements:
Active New York LPN license , in hand at the time of submission.
Current BLS certification .
Minimum 1 year of relevant experience in home health, case management, care coordination, or an outpatient clinic.
Hands-on Epic experience is mandatory.
Experience with patient follow-up, medication reconciliation, telephone triage, and care coordination.
Strong communication, documentation, organization, and patient education skills.
Ability to work independently and collaborate with an interdisciplinary care team.
Reliable transportation for potential home, hospital, and provider office visits; transporting patients is not required.
Willingness to float between Rochester General Hospital, Unity Hospital, patient homes, and provider offices.
