Position Overview:
The Outreach Medical Assistant plays a vital role in advancing the quality of care and promoting population health initiatives within our large health system's Department of Population Health. This position is primarily responsible for conducting outreach calls to patients with the goal of closing quality care gaps, improving healthcare outcomes, and enhancing patient engagement. Additionally, the Outreach Medical Assistant will assist in other outreach initiatives, including contacting non-users and high-utilizer patients, to promote access to care and support comprehensive population health management efforts.
Key Responsibilities
1. Patient Outreach and Engagement
o Conduct outbound phone calls to patients to assist patients with obtaining appointments and services to address quality care gaps, including missed preventive screenings, overdue vaccinations, and gaps in chronic disease management.
o Engage with patients in a respectful and empathetic manner to understand their healthcare needs, preferences, and barriers to accessing care.
o Assist newly assigned patients with making appointments to receive necessary medical services.
o Conduct outreach patients by email and mail communication methods as needed to help patients receive medical care.
2. Quality Care Gap Closure
o Work collaboratively with healthcare providers, care teams, and support staff to coordinate appointment scheduling with the goal of closing identified quality care gaps in a timely manner.
o Educate patients about the importance of preventive care, recommended screenings, and evidence-based interventions to improve their health outcomes and overall well-being.
3. Documentation and Reporting
o Document all outreach activities, patient interactions, and care coordination efforts accurately and comprehensively in the electronic health record (EHR) system.
o Generate reports and maintain data records to track outreach outcomes, measure the effectiveness of interventions, and report on progress towards quality improvement goals.
4. Non-User and High-Utilizer Outreach
o Assist in outreach efforts targeting non-user patients who have disengaged from healthcare services, encouraging them to re-engage with primary care and preventive services.
o Conduct outreach calls to high-utilizer patients to identify underlying healthcare needs, address social determinants of health, and facilitate appropriate care coordination to reduce avoidable healthcare utilization.
5. Patient Navigation and Support
o Facilitate communication between patients and healthcare providers to ensure continuity of care, address care coordination challenges, and promote patient-centered care delivery.
6. Other duties as assigned but that are aligned with the core mission of the population health department. Please note this position is not a hands on with patients position, and is strictly administrative.
