CLASSIFICATION: Hourly, Non-Exempt

LOCATION: Residential Care Department

JOB SUMMARY: Responsible for maintaining patient files and checking patient's insurance eligibility, responding to requests for medical records, and other clerical duties.

ACCOUNTABLE TO: Residential Care Management

JOB DUTIES: Responsible for maintaining files and checking patient's insurance eligibility including the following:

  1. Organize, file, and retrieve patient medical records.

  2. File various medical documentation through our EMR system, including but not limited to patient notes, radiology reports and lap results.

  3. Maintain patient admissions and discharges from various EMR systems

  4. Provide secretarial support to clinical staff.

  5. Responsible for entering patient charges due into EMR system

  6. Maintain over 550 patient rosters on daily basis

  7. Verify insurance eligibility for scheduled appointments under various EMR systems for over 250 patients daily.

  • Scan in all patient data, reports upon receipt of information.
  • Create charts for new patients and ensure consents get collected.
  1. Ensure all provider-patient visits are scheduled correctly into our EMR system.

  2. Provide monthly provider reports to over 550 licensed Board and Cares

  3. Transcribe patient medical notes to various EMR system that license Board and Cares utilize

  4. Answer telephone calls promptly and courteously

  5. Assist in compiling data and information for reports required by external agencies and regulatory authorities.

  6. Handle protected health information in a manner consistent with HIPAA (Health Insurance Portability and Accountability Act).

  7. Keep supervisor informed of problems or issues and suggest solutions.

  8. Consistently demonstrate and uphold CCCHC's principle of providing quality health and human services to the medically underserved and low-income populations in a culturally sensitive manner.

  9. Punctuality and Attendance: This is an essential job duty for CCCHC's employees given the impact on patients.

  10. Additional duties as assigned.

Special Knowledge, Skills, Abilities and Attributes:

  • Ability to maintain records and files in alphabetic/numeric sequence
  • Ability to operate personal computer and utilize Microsoft Word, Excel, and Outlook.
  • Ability to exert physical effort maintaining and distributing files.
  • Demonstrated ability to exercise sound judgment.
  • Ability to communicate clearly and concisely in person and over the phone with proper etiquette.
  • Ability to plan and be organized.
  • Ability to work well under pressure, take initiative and be flexible and cooperative.
  • Ability to work effectively with both employees and managers.
  • Ability to convey a positive and professional image.
  • Required to know, follow and enforce safe work practices, and be aware of company policies and procedures related to job safety, including safety rules and regulations.
  • Ability to use a computerized patient system (EMR)

Education and Experience Requirements

  • High school diploma or GED required.
  • Two (2) years of medical records experience preferred.
  • General medical office experience preferred.
  • Knowledge of various Health Insurances preferred

Working Conditions

Normal office environment with:

  • Sitting for long periods of time, routinely standing, walking, reaching with hands and arms, repetitive motions, hearing, seeing, speaking, feeling, stooping, and kneeling.
  • Ability to work at a computer terminal for extended periods of time on a daily basis.
  • Ability to exert up to 40 pounds of force in order to lift, carry, pull or move objects.
  • Ability to handle multiple tasks while also being tactful, diplomatic, and empathetic.
  • Ability to organize and schedule work effectively.
  • Ability to communicate effectively and tactfully with customers, managers and other levels of personnel, in person and on the telephone.
  • Ability to pay close attention to detail.
  • Ability to maintain composure under conditions of urgency and/or time constraint and meet deadlines.
  • Ability to handle and maintain patient and company information confidentiality.
