Position Overview
We are seeking a compassionate and experienced Palliative Registered Nurse (RN) to join our growing team. As a Palliative RN, you will provide skilled nursing care, comprehensive patient assessments, symptom management, and care coordination for patients living with chronic or life-limiting illnesses. Unlike hospice care, our focus is on improving quality of life while patients continue receiving curative or life-prolonging treatments when appropriate.
This position is ideal for a nurse who enjoys building meaningful relationships, educating patients and families, and collaborating with physicians and interdisciplinary team members to deliver exceptional home-based care.
Key Responsibilities
- Perform comprehensive in-home nursing assessments and ongoing patient evaluations.
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
- Monitor symptoms, provide clinical interventions, and communicate changes in patient condition to providers.
- Educate patients and caregivers on disease processes, medications, symptom management, and available community resources.
- Coordinate care with physicians, specialists, therapists, social workers, and other healthcare professionals.
- Advocate for patient goals and preferences while promoting comfort, independence, and quality of life.
- Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
- Participate in interdisciplinary team meetings and case conferences.
- Participate in the on-call rotation as scheduled.
Qualifications
Required:
- Graduate of an accredited school of nursing.
- Current and unrestricted Registered Nurse (RN) license in the State of New Mexico.
- Minimum of one year of nursing experience.
- Current driver's license, reliable transportation, and proof of automobile insurance.
- Strong assessment, communication, and critical thinking skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
Preferred:
- Experience in palliative care, home health, hospice, or case management.
- Experience with electronic health record (EHR) systems.
- Knowledge of chronic disease management and community resources.
