Are you an experienced RN ready to step away from direct field visits and into a pivotal leadership role? Our client is seeking a Patient Care Manager (PCM) to lead their clinical team in Philadelphia.
As the "heartbeat" of the office, you will oversee the coordination of care, ensure strict regulatory compliance, and mentor a multidisciplinary team of field clinicians. This is a high-impact role where your clinical expertise ensures that every patient receives the right care at the right time.
Key Responsibilities
Clinical Oversight: Supervise the daily clinical operations of a multidisciplinary team (RNs, LPNs, PTs, OTs).
Case Management: Review and approve plans of care, ensuring they are medically necessary and tailored to patient needs.
Compliance & Quality: Audit clinical documentation (OASIS) to ensure accuracy, timely submission, and adherence to CMS and state regulations.
Mentorship: Provide clinical guidance and education to field staff, assisting with complex cases or "red flag" clinical situations.
Utilization Review: Monitor patient progress and utilization of services to ensure optimal outcomes and resource management.
Collaboration: Act as the primary liaison between physicians, families, and field staff to facilitate seamless transitions of care.
Qualifications
Licensure: Active, unencumbered Registered Nurse (RN) license in the state of Pennsylvania.
Experience: Minimum of 2–3 years of home health experience is required.
Leadership: Previous experience in a supervisory or management role (PCM, Clinical Supervisor, or Team Lead) is strongly preferred.
Expertise: Deep understanding of OASIS-E and Medicare Conditions of Participation (CoPs).
Skills: Exceptional organizational skills and the ability to thrive in a fast-paced office environment.
