Responsibilities

  • Provide evidence-based physical therapy assessments and treatments for patients.

  • Develop and implement individualized therapy plans with measurable goals.

  • Maintain accurate documentation for all sessions and outcomes.

  • Meet or exceed weekly productivity targets and visibility metrics.

  • Adhere to safety, compliance, and professional standards in all activities.

  • Monitor patient progress and adjust plans to optimize outcomes.

  • Educate patients and families on home exercise programs and self-management.

  • Collaborate with interdisciplinary team to coordinate care and referrals.

  • Engage in ongoing professional development to advance clinical skills.

  • Document and communicate treatment plans and outcomes to stakeholders.

  • Proactively identify opportunities for service expansion within the territory.

  • Represent the clinic professionally in on-site visits and community outreach.


Experience/Qualifications

  • Licensed physical therapist with current applicable state license

  • Evidence-based assessment and treatment planning for PT patients

  • Develop and implement individualized therapy plans with measurable goals

  • Accurate documentation of sessions, outcomes, and progress notes

  • Safety, compliance, and professional standards adherence

  • Meet or exceed weekly productivity targets and visibility metrics

  • Monitor patient progress and adjust plans for optimized outcomes

  • Educate patients/families on home exercise programs and self-management

  • Collaborate with interdisciplinary team for care coordination

  • Engage in ongoing professional development and Skill advancement

  • Maintain punctual on-site presence for 3 days weekly with reliable attendance
