Physical Therapist

1. He/shemay beresponsibleforsupervising/trainingpersonnelsuchasPTA orPT Aides
2. Followingreceiptofadoctor'sreferral,thetherapistisresponsibleforevaluatingthe patient within 24 hours of written referral.
3. Afterevaluating thepatient,thetherapist isresponsible forplanning, administering and supervising the PTA in an appropriate treatment plan.
4. TheStaffphysicalTherapistwillcommunicate withthereferringphysicianand membersofthe rehabilitationteam regarding thepatient's total treatment program.
5. TheStaffPhysicalTherapistwill maintainanaccurate dailyrecord oftreatments given to patients.
6. TheStaffPhysical Therapist willprovide in-service educationtofacilities, supportive staff, students and community.
7. The StaffPhysicalTherapist willperformotherdutiesasassignedbytheArea Administrator.
8. TheStaffPhysicalTherapistwillfollowestablishedpoliciesandproceduresofthe company.
