Job Description:
We are seeking a dedicated Physical Therapist to support a skilled nursing and post-acute rehabilitation center serving a diverse adult and geriatric population in Bakersfield, CA. You will conduct comprehensive evaluations, design personalized plans of care, and implement treatments aimed at restoring mobility, balance, and functional independence. The facility provides short-term rehab and long-term care services and is supported by an interdisciplinary team of nursing and therapy professionals. This position offers a consistent schedule, a stable caseload, and a collaborative clinical environment.

Responsibilities

  • Conduct patient assessments to develop personalized treatment plans.

  • Implement therapeutic exercises and gait training to improve mobility and strength.

  • Set and achieve personal performance and productivity targets.

  • Monitor patient progress and adjust treatment plans as necessary.

  • Participate in ongoing professional development and training opportunities.

  • Educate patients and families on therapy goals, safety, and home programs.

  • Maintain accurate, compliant, and timely documentation of patient treatments.

  • Utilize evidence-based practices to enhance treatment effectiveness.

  • Collaborate closely with nursing, physicians, and other therapists to optimize patient outcomes.

  • Stay updated on best practices and advancements in SNF-based physical therapy.

  • Contribute ideas for process improvement and patient satisfaction initiatives.

Experience / Qualifications

  • Valid state license to practice as a Physical Therapist.

  • Prior experience in a SNF or post-acute rehab setting preferred, but not required.

  • Demonstrated ability to design and implement individualized treatment plans.

  • Strong clinical judgment and problem-solving skills.

  • Excellent communication and interpersonal skills with patients, families, and staff.

  • Proficiency with EMR and timely documentation.

  • Ability to manage a full-time caseload safely and efficiently.
