Job Description
Title: Physical Therapist - Home Health
Schedule: Mon - Fri
Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 30
Requirements:
- Currently licensed as a Physical Therapist in the state of practice
- Graduate of an accredited physical therapist curriculum
- Two years of clinical experience preferred; home health experience a plus
- Current CPR certification
- Basic computer proficiency; willingness to learn EMR systems and OASIS documentation
Roles & Responsibilities:
- Provide physical therapy services within the legal scope of practice, maintaining valid licensure and certifications
- Conduct evaluations and document progress in accordance with Medicare and state standards
- Develop and implement individualized plans of care aligned with patient needs
- Deliver skilled interventions and maintain therapy programs as permitted
- Educate patients and caregivers on exercise programs and adaptive strategies
- Supervise Physical Therapist Assistants and Home Health Aides in compliance with regulations
Compliance / Notes:
- Adhere to infection control, safety, and risk prevention protocols
- Maintain compliance with HIPAA and all privacy standards
