Responsibilities

  • Assess patients’ physical conditions and develop individualized treatment plans.

  • Provide evidence-based therapeutic interventions to improve mobility and function.

  • Document patient progress and update plans to reflect measurable outcomes.

  • Set and monitor individual performance targets aligned with pipelines.

  • Demonstrate continuous learning through ongoing professional development.

  • Ensure compliance with regulatory guidelines and clinic policies.

  • Utilize evidence-based practice to optimize treatment protocols.

  • Collaborate with physicians and care teams to coordinate care.

  • Educate patients and families on home programs and preventive strategies.

  • Maintain accurate scheduling and utilization of therapy resources.

  • Participate in daily huddles and provide visibility into caseload and outcomes.

  • Contribute to quality improvement initiatives to enhance patient care.


Experience/Qualifications

  • Licensed Physical Therapist with valid state license.

  • Develop and document individualized, evidence-based treatment plans.

  • Demonstrate measurable patient outcomes and update plans regularly.

  • Adhere to regulatory guidelines, clinic policies, and professional standards.

  • Collaborate with physicians and care teams to coordinate care.

  • Educate patients and families on home programs and preventive strategies.

  • Document progress and performance targets aligned with pipelines.

  • Engage in ongoing professional development and continuous learning.

  • Maintain accurate scheduling and optimal utilization of therapy resources.

  • Participate in daily huddles and provide caseload visibility.
