    Job Description

About Jordan CRC / Phoenix Counseling Solutions

Jordan CRC and Phoenix Counseling Solutions provide behavioral health, substance use treatment, recovery housing, employment services, case management, and other supportive services for women working toward recovery and long-term stability.

Our approach is trauma-informed, person-centered, and recovery-oriented. We work closely with community partners to help women overcome barriers related to treatment, employment, housing, and recovery.

Jordan CRC is accredited by The Joint Commission and certified by the Ohio Department of Mental Health and Addiction Services (OhioMHAS) and the Ohio Recovery Housing (ORH) network.

    About the role

We're hiring a full-time Project Coordinator to run the day-to-day work of our HIV/AIDS Project. Your job is to make sure no participant gets lost between outreach, intake, treatment, HIV-related services, and recovery support.

Working closely with the Project Director, you'll guide participants from first contact through screening, enrollment, assessment, and service planning, then connect them to substance use, behavioral health, HIV prevention and testing, and recovery services. You'll be the central link among participants, clinicians, outreach staff, community partners, and project evaluators, using trauma-informed, culturally responsive, recovery-oriented care.

    What you'll do

Coordinate the project day to day

    • Partner with the Project Director on goals, timelines, and deliverables.

    • Manage participant flow from outreach and referral through intake, service planning, treatment, follow-up, and discharge.

    • Track engagement, referrals, missed appointments, barriers, and service gaps.

    Screen, enroll, and plan services

    • Coordinate or conduct screening, eligibility, and intake within your scope of practice, including SUD, mental health, trauma, and HIV-risk screening.

    • Make sure consents, releases, and eligibility documents are complete.

    • Coordinate clinical assessments and Individualized Service Plans that reflect each participant's strengths, needs, and goals.

    • Recognize urgent needs and connect participants to qualified crisis, medical, or behavioral health providers.

    Connect participants to care

    • Coordinate access to SUD, behavioral health, and integrated co-occurring treatment.

    • Integrate HIV prevention, education, testing, and linkage to medical care into participant services.

    • Arrange warm handoffs to internal and external providers and track referrals through completion.

    Build community partnerships

    • Support outreach, recruitment, community presentations, and testing events.

    • Develop referral relationships with treatment providers, HIV/AIDS service organizations, hospitals, housing, justice-system, and faith-based partners.

    • Re-engage participants who become disconnected from services.

    Collaborate, document, and measure

    • Participate in treatment-team meetings, case staffing, and quality improvement.

    • Work with evaluators on grant-required data collection, performance monitoring, and reports.

    • Keep timely, accurate records in line with HIPAA, 42 CFR Part 2, and funder requirements.

    What we're looking for

    • Bachelor's or master's degree in social work, counseling, addiction studies, psychology, behavioral health, human services, public health, or a related field.

    • Ohio LSW and/or LCDC III strongly preferred; other behavioral health or SUD credentials considered.

    • 3+ years in substance use treatment, behavioral health, co-occurring disorders, treatment coordination, case management, intake, or related human services.

    • Experience with screening, assessments, treatment plans, ISPs, referrals, and trauma-informed care.

    • Experience with outreach, referral partnerships, multidisciplinary teams, and diverse or underserved populations.

    Nice to have

    • HIV/AIDS prevention, testing, treatment linkage, or public health experience.

    • Experience with federally funded behavioral health or SUD programs.

    Schedule & pay

This is a full-time position based at Jordan CRC / Phoenix Counseling Solutions, with regular work at partner agencies, community locations, and outreach sites. The salary range is 45,000–60,000 per year.
