Psychiatric Rehabilitation Services Coordinator

RYZE on the Avenue is seeking a dedicated Psychiatric Rehabilitation Services Coordinator (PRSC) to coordinate and oversee psychiatric rehabilitation services for residents with behavioral health and psychosocial needs. The PRSC will work closely with the interdisciplinary team to support individualized treatment planning, promote independence, and help residents develop the skills needed to improve their quality of life.

Duties Responsibilities

• Coordinate and oversee psychiatric rehabilitation services in accordance with resident needs, care plans, and facility policies.
• Assist with developing, implementing, and monitoring individualized psychiatric rehabilitation plans.
• Work collaboratively with nursing, social services, therapy, and other interdisciplinary team members to support resident goals.
• Help residents develop daily living, communication, social, coping, and independent living skills.
• Monitor resident participation, progress, and response to rehabilitation services.
• Maintain accurate and timely documentation of services, resident progress, and treatment goals.
• Support residents in developing strategies to manage behavioral health challenges and improve daily functioning.
• Coordinate therapeutic activities and rehabilitation interventions based on individual needs and abilities.
• Communicate with residents, families, and care team members regarding progress and ongoing support needs, as appropriate.
• Assist with discharge planning, community resource coordination, and transitions to appropriate levels of care.
• Ensure services are delivered in accordance with applicable regulations, professional standards, and facility policies.
• Participate in interdisciplinary meetings, care plan reviews, and quality improvement initiatives.
• Maintain confidentiality of resident information and comply with HIPAA requirements.
• Perform other duties as assigned.

Qualifications

• Bachelor’s degree in Psychology, Social Work, Behavioral Health, Rehabilitation Services, or a related field preferred.
• Experience in psychiatric rehabilitation, behavioral health, mental health services, or a related field preferred.
• Experience working with individuals with psychiatric or psychosocial needs strongly preferred.
• Knowledge of rehabilitation principles, behavioral health support, and individualized service planning.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively as part of an interdisciplinary team.
• Strong documentation and time-management skills.
• Commitment to resident-centered care, dignity, and promoting independence.
