The Evidence Based Care Coordinator RN provides ongoing support and expertise through assessment, planning, implementation and evaluation. The overall goal of the position is to enhance the overall quality of patient care and outcomes through providing successful achievement of reported metrics, including Core Measures, Sepsis and Stroke, adding metrics as deemed appropriate. Responsible for the ongoing performance improvement for metrics through data review and analysis, implementation of strategies that will provide successful improvement of evidence based clinical approaches and care delivery. Responsible for establishing working relationships with physicians and staff through collaboration and care coordination for holistic care delivery which will produce superior health outcomes. Responsible for coordinating presentation of metrics to applicable committees and facility and division groups. Responsible for continued learning/education for all Healthcare Quality Metrics and responsible for transfer of knowledge through frequent communication with physicians, nursing, management and patient care staff. Fosters teamwork through support and accountability.

GENERAL RESPONSIBILITIES:

· Possesses leadership skills in all health care services

· Demonstrates ability to create, initiate and integrate new concepts and processes/practices to facilitate improvements

· Demonstrate ability to create and manage Microsystems of care

· Demonstrates ability to successfully manage multiple tasks and projects in a productive manner

· Demonstrate skill in knowledge acquisition, management of change and dissemination of new knowledge to healthcare professionals

· Educates staff, leaders and physicians in metric requirements and evidence based care requirements

· Demonstrates interdepartmental support and fosters teamwork in a collaborative manner

· Applies analytical approach to problem solving

· Supports patient care staff through visibility and fostered relationships

· Demonstrates ability to transform care at the bedside through transfer of knowledge related to population appropriate health care to individuals

· Ability to communicate positive results to staff and communicate to managers staff who are identified with negative results

· Knowledgeable of requirements for all core measures and HCA core measure initiatives related to measured metrics.

· Functions as the facility Stroke Coordinator and Sepsis Coordinator

· Coordinates activities for Joint Commission Primary Stroke Center certification

· Maintains knowledge base and understanding of core measure specifications and data elements, including but not limited to inclusion and exclusion criteria, algorithms and reporting of any changes within the measures

· Attends education offerings by state QIO, HCA and Comet

· Oversight in conjunction with CNO and VP Quality of all PI activities related to patient care outcome reporting including core measures, sepsis and stroke

· Completes intensive analysis of all core measure metric variances and works with nursing/patient care staff to develop measureable action plans.

· Participate with division concurrent workgroup to standardize practices across all healthcare settings

· Accountable for evaluation and improvement in point of care outcomes

· Other duties as assigned

EDUCATION/EXPERIENCE:

· Graduate of accredited School of Nursing

· Current TN state RN license

· BSN required, MSN preferred

· Current BLS Certification upon hire

· Previous clinical experience with a minimum of 3 years.

· Excellent verbal and written communication skills

· Knowledge of systems understanding, performance improvement tools/methodology and facilitation/teaching skill
