About the Role

We're seeking an experienced Registered Nurse Care Coordinator to support our Collaborative Care and Chronic Disease Management programs.

In this role, you'll serve as a patient advocate and clinical resource, partnering with providers and interdisciplinary teams to coordinate care, improve outcomes, and empower patients to successfully manage chronic conditions.

Schedule:

Monday - Wednesday | 9:00 AM - 6:00 PM

Thursday & Friday | 8:00 AM - 5:00 PM

What You'll Do

  • Coordinate care and advocate for patients with chronic conditions

  • Assess patients and develop individualized care plans

  • Educate and coach patients to improve self-management

  • Lead interdisciplinary care team meetings

  • Collaborate with providers and clinical teams to improve outcomes

  • Identify high-risk patients using population health tools

  • Document patient care plans and maintain accurate EMR documentation

  • Support referrals, care transitions, and quality improvement initiatives

What You'll Bring

Required

  • Bachelor's degree in Nursing

  • Active RN license in good standing (multistate preferred)

  • Current CPR certification

  • Five or more years of nursing experience

Preferred

  • Care coordination, case management, chronic disease management, or population health experience

  • Motivational interviewing experience

  • Strong communication, organization, and critical thinking skills

  • Experience with EMRs and Microsoft Office
