RN Case Manager

Mohawk Valley Health System is seeking a compassionate RN Case Manager to coordinate and optimize patient care across the continuum. In this role, you will conduct assessments, develop and implement care plans, collaborate with providers and community resources, and support safe, timely discharges. You’ll advocate for patients and families, monitor progress, and ensure appropriate use of services.
