Shift: Days, Monday–Friday (8:00 AM – 5:00 PM) Schedule: 8-hour shifts, No weekends, No holidays Contract: 13 Weeks Hours: 40 hours/week
Requirements
Active RN license
Minimum home health experience
Homecare Homebase (HCHB) experience required
BLS certification
Valid driver's license and auto insurance
Experience with:
Wound care
Wound VACs
Ostomy care
PICC line care
IV antibiotics
Lab draws
CHF and pneumonia patients
Responsibilities
Provide routine skilled nursing visits in patients' homes.
Assess and manage wound care, wound VACs, and ostomies.
Perform PICC line care, IV therapy, and lab collection.
Educate patients and caregivers on disease management and medications.
Document patient care using Homecare Homebase (HCHB).
Collaborate with physicians and the interdisciplinary care team.
Schedule & Work Environment
Up to 5 routine patient visits per day
No Start of Care (SOC), recertifications, or case management
No on-call
No floating
One-week orientation with a preceptor
Samsung tablet and company cell phone provided
Casual business attire or clean scrubs acceptable
Required Certifications
Active RN License
BLS Certification
Valid Driver's License
Current Auto Insurance
