Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Roles & Responsibilities
- Provide direct patient care in patients' homes, focusing on wound care management including wound vacs and ostomy care
- Monitor and manage cardiac patients with CHF, pneumonia, and other conditions
- Administer IV antibiotics and perform PICC line care and lab draws
- Collaborate with the healthcare team to ensure high-quality care
- Maintain accurate documentation using Homecare HomeBase EHR on a Samsung tablet
- Educate patients and families on disease management and treatment plans
Knowledge & Skills
- Knowledge of home health regulations and OASIS
- Skills in wound assessment and care
- IV therapy and PICC line management
- Effective communication and patient education
- Time management and ability to work independently
