Description:
This position involves providing home care and hospice services primarily for a geriatric population, with occasional pediatrics.
Common diagnoses include heart disease, chronic respiratory diseases, congestive heart failure, chronic obstructive pulmonary disease, pneumonia, dementia/Alzheimer's, depression, stroke, cancer, kidney disease, diabetes, liver disease, wounds, joint replacement, surgery, and medication management.
The RN will typically see 5-6 patients per day, totaling 25-30 visits per week, which include routine and OASIS visits.
Additional job duties include attending weekly interdisciplinary team meetings and case conferences.
There is no use of mechanical ventilation, but CPAP and BiPAP devices may be utilized by patients for self-management.
The position requires critical thinking and effective communication skills, especially focused on educating patients and families.
Hospice experience should include the management of pain and symptoms, along with providing end-of-life care.
Nursing staff will work with certified nursing assistants in the home health and hospice settings.
The schedule is Monday through Friday from 8:00 AM to 4:30 PM, with occasional on-call weekend shifts.
The RN will be expected to adhere to a professional dress code, wearing solid color scrubs and closed-toe, non-slip shoes.
Field training and shadowing will be provided to familiarize the RN with department-specific procedures.
All clinicians must clock in and out for record-keeping.
