Shifts: Monday thru Friday 8:00am-4:30pm, with variable end time
Seven Healthcare are seeking an experienced RN specializing in Home Health for a travel assignment in Beaver Dam, WI. This role involves providing quality care to a primarily geriatric population within a post-acute setting, covering both home health and hospice visits. Key Responsibilities of the RN - Home Health- Provide home care and hospice services for chronic disease management and patient/family education
- Manage a caseload of 5-6 patients per day, including routine and OASIS visits
- Participate in weekly IDT meetings and monthly all staff meetings
- Utilize skills in pain and symptom management, end-of-life care, and complex wound care
- Perform NPWT, ostomy care, central line care, and IV administration
Requirements of the RN - Home Health- Active RN license with travel experience
- Certification in BLS is required
- Proven experience in chronic disease management and hospice care
- Proficiency in Cerner charting system
- Skills in critical thinking and complex wound care management
