About the Position

Job Summary:

We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality patient care in home health and hospice settings. The RN will deliver direct patient care, coordinate services, and manage cases to ensure continuity and optimal outcomes. This role involves working closely with patients, families, and an interdisciplinary team to support physical, emotional, and psychosocial needs.

Key Responsibilities:

• Provide skilled nursing care to patients in their homes or inpatient hospice settings.
• Perform patient assessments, develop care plans, and implement nursing interventions.
• Monitor and evaluate patient conditions, including responses to treatments and medications.
• Coordinate care with physicians, healthcare teams, and community resources.
• Supervise and guide Home Health Aides and CNAs to ensure quality care delivery.
• Ensure continuity of care through effective communication and case management.
• Educate patients and families on disease management, medications, and care plans.
• Maintain accurate and compliant patient documentation (including OASIS requirements).
• Participate in on-call rotation and respond to patient needs via phone or home visits.
• Support quality improvement initiatives and attend required training sessions.
• Perform additional duties as assigned.

Requirements

• Active Registered Nurse (RN) license in the State of Montana
• Minimum 2 years of recent experience in acute care, home health, or hospice
• Strong clinical skills including:
  • Venipuncture
  • Catheterization
  • Wound care
  • Parenteral therapyDe
