Job Summary:
Long Term Care (LTC) Nurse specializing in elderly and rehabilitation care is needed at a healthcare facility in Beaver Dam, Wisconsin. The position involves working in a nursing home environment with a focus on dementia, frail, and elderly patients. The role includes charting, assessments, charge duties, medication administration, and clinical skills such as IV therapy, catheterization, and ostomy management. The assignment offers a comprehensive clinical experience within a multidisciplinary team setting.

Job Details:

  • Schedule: Day shifts from 10:30 AM to 11:00 PM; minimum of 36 hours per week.
  • Required Qualifications: Active BLS certification through the American Heart Association; minimum of 1 year of elderly experience; eligible to submit license; must have worked with Marshfield at least 6 months prior if applicable.
  • Preferred Qualifications: None specified.
  • Float: Float rotation may be required.
  • Additional Information:
    • Located at a nursing home attached to a hospital with 70 beds, majority private rooms.
    • EMR system in use is ECS.
    • Nurse to patient ratios are 1:20 for RNs and LPNs, 1:8 for CNAs.
    • Clinical duties include IVs, catheters, ostomy care, and titratable drips.
    • Interdisciplinary team includes therapy, administration, social workers, activities, dietary, and housekeeping.
    • On-site RT 24/7, pharmacy equipped with medication dispensing system.
    • No on-call requirement; holiday availability required for half of holidays.
    • Scrub color is any solid color, clean, and professional.
    • Clinicians must clock in and out at the facility on each shift.
    • The start date for this assignment is 08/10/2026.
