Job Title: Wound, Ostomy and Continence Nurse Profession: Nursing Specialty: Registered Nurse Duration: 13 weeks Shift: Days Hours per Shift: 5x8 Experience: Three to five years in healthcare License: Active NYS Registered Nurse license Certifications: Certified Wound Ostomy Nurse (CWON), Certified Wound Care Nurse (CWCN), Certified Wound Specialist (CWS), or Wound Care Certified (WCC) Must-Have: Wound Care Certificate Description: Under the direction and supervision of the Wound and Ostomy Manager, the wound, ostomy, and continence nurse will provide safe, competent, quality care based on nursing theory and research to a designated group of patients. The nurse will conduct health assessments focused on wound, ostomy, and continence-related care. They will perform focused physical assessments and develop plans of care based on diagnoses and individual client needs. Recommendations for treatments will be made as appropriate. There will be assistance with the development, revisions, and evaluations of standards of care. Assessment and documentation of patient responses to wound care, ostomy, continence, fistula, and complex wound treatments will be conducted. The nurse will evaluate the quality and delivery of care provided by the clinical staff. Collaboration with the healthcare team will occur, and referrals to other disciplines will be made as indicated. Teaching, counseling, and assisting individuals and families to assume responsibility for the prevention of complications related to patients with ostomies, wounds, or continence problems will be a key responsibility. Routine comprehensive assessments of patients at risk for developing pressure injuries will be provided, along with the initiation of preventative measures. Participation in unit and department quality improvement and hospital-wide activities, including incidence and prevalence studies, will be expected. The nurse will provide culturally competent patient, family, and staff education consistent with cultural, ethnic, religious, and other belief systems. Maintaining the security and confidentiality of data and information in accordance with patient rights policies will be essential. Additional tasks will be performed as assigned. Flexible hours to support patient-centered care may be required. Hands-on experience and experience with pressure injuries are necessary. Good interpersonal skills are essential for success in this role.
