The White Oak Post Acute is seeking a dedicated Wound Care/Treatment LPN! 
Registered Nurse (LPN or RN)- Treatment Focus
MUST HAVE WOUND CARE EXPERIENCE!!! 

Responsibilities:
  • Patient Assessment & Care: Perform comprehensive assessments, monitor vital signs, and document resident conditions, reporting changes to physicians and unit managers.
  • Wound Care Specialist: Manage all aspects of wound care, including assessing skin breakdown, applying treatments, educating staff/residents, documenting wound status, and completing quality assurance reports.
  • Medication & Treatment Administration: Administer medications and treatments as ordered, transcribing physician orders accurately.
  • Care Plan Management: Implement and coordinate care plans, ensuring adherence to physician orders, facility policies, and federal/state guidelines.
  • Supervision & Delegation: Oversee and guide nursing assistants (CNAs) in providing care, ensuring proper skin prevention programs and daily tasks are completed.
  • Documentation: Maintain accurate, detailed, and timely clinical records and reports.
  • Communication: Act as a liaison between residents, families, physicians, and the interdisciplinary team.
  • Emergency Response: Provide prompt response to urgent physical and emotional needs and manage emergency situations.
  • Education: Educate residents and families on self-care techniques and treatment plans. 
Qualifications:
  • Current, valid LPN license.
  • Experience in long-term care or SNF setting preferred.
  • Strong assessment, critical thinking, and organizational skills.
  • Ability to work independently and as part of a team. 
Key Skills:
  • Wound care expertise (dressings, assessment).
